Understanding 'Expired' in Medical Records: Medications & Patient Status
Expired has two different meanings in medical records:
1. Expired medications: Medications, like all consumables, have an expiration date. As medications can lose their potency once expired, a notation of EXP on a medical record will be made when there is an expired dosage form/quantity on file/in supply at a dispensing unit/pharmacy
2. A patient has deceased: In this case, "expired" simply indicates a person's status of being deceased in hospital.
