Alert: The person is awake and alert. They can respond to questions and follow instructions.Vocal: The person is responsive to verbal stimuli. They can make sounds or words, but they may not be able to speak clearly.
Painful: The person responds to painful stimuli. They may cry out or move away from the source of pain.
Unresponsive: The person does not respond to any stimuli. They are unconscious and unresponsive.
