Which appendix in the cpt manual has complete list of modifier -51 exempt codes?

There is no list of codes exempt from the requirements for modifier -51 in the CPT Index or its appendices. There are, however, lists of codes that are exempt from the requirements of specific global surgery rules found in Appendix F of the CPT manual.
