Patient: 75-year-old male who experienced a left-sided stroke 3 days ago.
Medical Diagnosis: Left-sided stroke
Nursing Diagnosis: Impaired Physical Mobility related to weakness on the left side of the body
Patient Outcomes:
* The patient will be able to perform active range-of-motion exercises on the left side of the body without pain.
* The patient will be able to transfer from bed to chair and back with minimal assistance.
* The patient will be able to ambulate with a walker for 100 feet without dyspnea or dizziness.
Nursing Interventions:
* Assist the patient with active range-of-motion exercises on the left side of the body 3 times per day.
* Provide the patient with a transfer belt and instruct him on how to use it to transfer from bed to chair and back.
* Encourage the patient to ambulate with a walker for 100 feet 3 times per day.
* Monitor the patient for dyspnea and dizziness during ambulation.
Evaluation:
The patient was able to meet all of the expected outcomes. He was able to perform active range-of-motion exercises on the left side of the body without pain, transfer from bed to chair and back with minimal assistance, and ambulate with a walker for 100 feet without dyspnea or dizziness.
