e. Food Handler's Certificate of the proprietor and 2 key personnel (Original and photocopy)
f. Medical Certificates of the proprietor and 2 key personnel (Original and photocopy)

h. Business TIN Number
i. Health Card from DOH (Original and photocopy)
j. Health Certificate of Business Establishment from DOH (Original and photocopy)
