FMLA Leave: Written Documentation Requirements & How to Request
The Family and Medical Leave Act (FMLA) does not require that a request for leave be in writing. An employee may make an oral request for leave. However, the employer may require the employee to provide written documentation supporting the need for leave, such as a doctor's note.
Here are some tips for requesting FMLA leave:

- Be clear about the dates of your leave and the reason for your leave.
