Phobia Hierarchies: Building Fear Ladders for Systematic Desensitization
Phobia hierarchies (fear ladders) are a core component of systematic desensitization, a behavioral therapy for specific phobias.
How They Work
- Identify the target fear: e.g., spiders, flying, needles.
- Generate 10–15 graded scenarios: From least to most anxiety-provoking (0–100 SUDS scale).
- Example (spider): 10 = see cartoon spider; 30 = photo; 50 = video; 70 = in jar across room; 90 = hold jar; 100 = spider on hand.
- Learn relaxation: Diaphragmatic breathing, progressive muscle relaxation, mindfulness.
- Gradual exposure: Start at lowest rung. Practice relaxation until SUDS drops ≥50%. Advance only when comfortable.
- Repeat across sessions: In vivo (real) or imaginal/VR exposure.
Key Principles
- Patient controls pace—therapist guides, never forces.
- Exposure must be prolonged (habituation) and repeated (extinction learning).
- Cognitive restructuring integrated: challenge catastrophic thoughts.
- Virtual reality (VRET) enhances access and gradation.
Effectiveness
High success rates (70–90%) for specific phobias. Also used in PTSD (prolonged exposure), OCD (ERP), social anxiety.
