Preinteraction phase

This is the phase before the nurse and the patient actually meet. The nurse may gather information about the patient from their medical records, and begin to develop a plan of care.

Orientation phase

This is the phase when the nurse and patient first meet. The nurse introduces themselves and explains their role, and the patient shares information about their health and concerns. The nurse begins to assess the patient's needs and develops a plan of care.

Working phase

This is the phase when the nurse and patient work together to implement the plan of care. The nurse provides care, education, and support for the patient, and the patient participates in their own care.

Termination phase

This is the phase when the nurse and patient end their relationship. This may occur when the patient is discharged from the hospital or when the nurse's assignment is completed. The nurse and the patient discuss the progress that has been made and any remaining needs or concerns. The patient is given a plan for continuing care.
