How to Find a Doctor by Medicare Provider Number
Medicare.gov allows you to find a physician or other provider by name, specialty or location, among other things. However, if you know your doctor's Medicare provider number (National Provider Identifier or NPI), and you can't remember his name, you can still contact him with billing or other questions. Another government site lets you find a doctor's name and contact information using just the doctor's provider number.Things You'll Need

Instructions
Open your web browser and go to the website for the National Plan and Provider Enumeration System (see References).
Click on "Search the NPI Registry"; then click "Search for an Individual Provider."
Enter the doctor's Medicare provider number in the "NPI" text box and click "Search."
