How to File a Dental Claim to Medical Insurance
Many dental offices will not bill medical insurance companies because medical insurance policies rarely offer dental coverage. However, many medical insurance policies do offer dental coverage for accidental dental injuries. This leaves many consumers forced to file their own dental claims when they experience a dental injury. Though this may seem daunting, the process is straightforward.

Contact the medical insurance company. Customer service phone numbers are usually listed on the back of the insurance card. Mention to the customer service representative that you have experienced a dental injury and will need to see a dentist for care. Ask for pre-authorization to have the dentist's services covered. This is an important step because many medical insurance companies will underpay or deny dental claims that are not pre-authorized.
Obtain a dental insurance form from your dentist on your first visit.
Obtain a copy of the bill for services from the dental office. Attach the bill to the completed dental insurance form.
Mail the bill and dental insurance form to your medical insurance company with delivery confirmation. Many medical insurance cards have the claims mailing address on the back of the card. If your card does not list the claims mailing address, call the customer service phone number and ask for the mailing address.
