Patient Identifiable Data (PID): Definition, Examples & HIPAA Compliance
Patient Identifiable Data (PID) : Includes but is not limited to:- First and last name.
- Full face digital photographs/video.
- Dates directly related to the individual including birth date and date of death when tied to a specific individual’s identity.
- Phone numbers, email addresses, and unique identifying numbers, characteristics or codes (except as specified in 45 C.F.R. 164.514(b)(2)(ii)(E)(1)).
- Medical record number.
- Health plan beneficiary number.
- Account numbers.
- Certificate / license Numbers.
- Vehicle identifiers and serial numbers, including license plate number.
- Device identifiers and serial numbers.
- Web URLs.
- Internet Protocol (IP) addresses when combined with other unique identifiers of the person.
- Full-face photographs and any comparable images.
- Any other unique identifying number, characteristic or code.
- Street Address, City, County, Province/State, and Postal/Zip code.
- Biometric identifiers, including fingerprints, retinal scans, voice prints, and full face photographic images and any other comparable images.
