Medical Records Filing: Order for Established Patient Charts

The medical assistant should file materials in the record of an established patient in the following order:

- Demographic information (name, address, phone number, etc.)

- Medical history

- Physical examination reports

- Laboratory test results

- Imaging studies

- Progress notes

- Hospitalization reports

- Consultation reports

- Operative reports

- Discharge summaries

- Correspondence (letters, emails, etc.)
