Medical Bill Denial? How to Write a Powerful Insurance Appeal Letter
Whenever you undergo a medical procedure, there is likely going to be a large medical bill attached. If you have medical insurance coverage, your insurance provider might pay for part of the procedure and you would pay for part of the bill out of pocket. But if your insurance provider denies your claim and will not pay the remainder of the bill, write an appeal letter to your insurance provider.
Instructions
Type your name and medical insurance policy number at the top of the appeal letter. Include any other identifying numbers, such as a group number or claim number, and your contact information.
State why your claim was denied according to the insurance provider.
Explain your medical condition and the procedure you had done, as well as reasons why the procedure was necessary.
Give specific information about why your claim should not have been denied. If you're not sure, ask your physician. The more detailed you can be, the better.
Tell the insurance provider you would like it to pay the remaining portion of the bill because the procedure was necessary for your health.
Add a deadline that you would like to hear back from the insurance provider.
Restate your contact information and explain that you are willing to provide any other necessary information.
