TennCare Card Replacement & Additional Card Request | Official Information

TennCare is one of the country's oldest Medicaid managed care programs offering pharmacy, medical, dental and behavioral care services. This program was launched in Tennessee in 1994, allowing all enrollees to enroll in their choice of a four managed care organizations. If you need to order additional TennCare cards, you have to contact TennCare's customer service department and request these cards by phone.

  Call customer service at 800-690-1606 to request additional cards from AmeriChoice. Contact AmeriGroup at 800-600-4441, TennCare Select at 800-263-5479 and BlueCare at 800-999-1658.

  Provide the customer service agent with identifying information. This information includes your date of birth, your first and last name, and your social security number.

  Request additional cards be mailed to you. Confirm your mailing address and wait for your additional cards to come in the mail.
