How to Register an NPI Number
Medicare and Medicaid Centers developed the National Plan and Provider Enumeration System to create specific identification information. The NPI registry provides an efficient option for the transmission of health information and provides consumers with an opportunity to search the NPI database to find physicians that accept Medicare or Medicaid. Knowing how to register an NPI number can help ease the registration process.
Instructions
Before you begin registering for an NPI, make sure that you have information about your business or organization, including an organization or provider name, tax identification number or Social Security number, authorized gender, number, date of birth, country, state, provider gender, mailing address, practice location, state licensing information, contact person, phone number, and email address.
Go to the National Plan and Provider Enumeration System website to submit your application. See the References section of this article for a link.
Read the terms and conditions in order to submit your application, including the privacy regulations, privacy act requirements, and penalties for the falsification of information.
Click ̶0;Apply Online for a National Provider Identifier̶1; and then go to the next web page. Go to ̶0;Begin Application Form.̶1; Create a user identification, by entering your Social Security number and password, then enter your desired secret questions and answers.
Enter your basic information, including the reason for your submittal form, and the type of business, such as sole proprietor or organization. Include your identifying information, such as the business owner's name, any former names, credentials, date of birth, gender, and the Social Security number or the tax identification number. Include your business mailing address, contact telephone numbers, and business locations. Include any provider identification numbers that you have received from Medicare or Medicaid, as well as license number and taxonomy codes. Make sure that the application is submitted with the practitioner̵7;s signature and contact person̵7;s information.
