How to Retrieve Immunization Records
Immunization records are important, especially when changing physicians and applying for jobs. Colleges and the military also request immunization information. Look in your old paperwork or important papers file to see if you already have this information. Otherwise, you will have to contact all clinics or physicians previously seen in order to get a full immunization history.
Instructions
Call your local health or state immunization program and ask if your immunization history is on file. According to the Centers for Disease Control, states which utilize an Immunization Information System (IIS) may keep this information.
Call or write to the health care provider who administered the vaccinations. The provider who gave you the vaccinations should have it documented in your medical records.
Identify yourself, date of birth and reason for the request. Your personal information is used to match your medical records.
Request an official copy of your immunization records be either mailed or faxed to you.
Repeat the same process with any other medical provider or clinic where you received a vaccination. Unless your records were transferred to your current provider, you will have to contact each clinic until you retrieve a full immunization history.
Contact colleges you may have attended and request any immunization records. Schools often administer vaccinations and keep that information on file.
Ask parents and any existing relatives if they either have or know where to find your immunization records.
