Child Medical Consent Form for Out-of-State Travel: Protect Your Minor
Planning ahead for a minor child to travel out of state with friends or family members includes writing a medical consent form that gives the other family permission to make medical decisions for the child. Hopefully, they will not experience a situation where the consent is necessary, but trying to execute the document over long distances can delay the child's receiving necessary medical treatment. There are several medical consent forms available online to use as a template.
Instructions
Write the medical consent form to include the following information.
In case of emergency I authorize (full name) of (full address) to give medical consent during my absence to (full name of child/children) for them to receive medical treatment from (beginning date) and (ending date). Alternatively, there are also medical consent forms available on the Internet if you would rather use a premade form.
List the child's full name, physician's name, allergies, blood type and pertinent medical history. Copy the child's insurance card and attach it to the consent form.
Sign the consent document and make a copy for your records. Send the original copy with the friends or family members caring for your child. It is not required to have the medical consent form witnessed by a notary public, though having the document notarized lends credence to its authenticity.
