A healthcare passport for a baby can help you keep track of all immunizations and visits with the doctor. A healthcare passport is a record of personal health information, including a patient's vital and insurance information, current conditions, medications, emergency contact information and many other aspects of a person's medical care. Request a personal healthcare passport booklet from your child's doctor or insurance company. Some hospitals will give newborns a healthcare passport booklet in the hospital to record all of their first immunizations.
Gather all of the baby's personal health information, records of immunizations, height and weight measurements from well-child checkups, and a brief physical description of the child, including any birth or identifying marks.
Enter the information into the healthcare passport booklet. If you are unable to get a healthcare booklet from your physician, insurance company or hospital, personal healthcare passport information forms are available to download online (see resources).
Take this medical record booklet to each physician visit to keep all of the information current.
