Coding Lesion Resolution: Understanding Code 3 & 4
Code 3: Lesion(s) not visible to the naked eye
- Code 3 is used for lesions confirmed or believed to be completely destroyed by the treatment of the lesion, but whose presence cannot be determined because they are not visible to the naked eye. Code 3 is rarely used.
Code 4: Lesions visible to the naked eye
- Code 4 is used when the lesion has not completely destroyed the lesion as determined by visual or endoscopic inspection.
