How to File an Extension for Disability
When you are on disability, it means that you can no longer work because of a physical or mental disability or issue. Depending on the disability, you will typically receive money to help you live your life during the time in which you cannot work. If your disability needs to be extended for whatever reason, you can usually do this with the government forms that you used to file disability in the first place.
Instructions
Go to the Social Security Administration's disability planning website (see Resources.)
Choose "File an Extension."
Fill out the form online, or print it and fill it out before sending it in. Fill out your name, your disability account number, the reason for your disability and the time frame. Then fill out your extension period, the reasons for the extension and whether or not the extension has been approved by your doctor. Depending on your case file and on your disability, you may need to include references or letters from your employer or your doctor.
