Inoperable Pancreatic Cancer: Symptoms, Palliative Treatment & Prognosis

Inoperable pancreatic cancer means the tumor cannot be surgically removed due to metastatic spread (Stage IV) or locally advanced vascular involvement (Stage III) precluding safe resection. Over 80% of patients present at an advanced stage.

Common Symptoms

• Epigastric or back pain (worse lying flat, better leaning forward).
• Unintentional weight loss, anorexia, early satiety.
• Jaundice (painless, obstructive) → dark urine, pale stools, itching.
• New-onset diabetes (due to insulin deficiency).
• Nausea, vomiting (gastric outlet obstruction).

Treatment Goals: Palliative, Not Curative

• Systemic Therapy:
  • FOLFIRINOX (5-FU/leucovorin/irinotecan/oxaliplatin) — for fit patients.
  • Gemcitabine + nab-paclitaxel — standard alternative.
  • Gemcitabine alone — for frail patients.
  • Targeted: Olaparib (BRCA mutation), pembrolizumab (MSI-H/dMMR), erlotinib (modest benefit).
• Radiation/Chemoradiation: Local control for locally advanced disease; pain palliation.
• Biliary/Gastric Obstruction: Endoscopic stent (plastic or metal) or surgical bypass.
• Pain Management: WHO analgesic ladder; celiac plexus neurolytic block for refractory pain.
• Pancreatic Enzyme Replacement: For exocrine insufficiency (steatorrhea, weight loss).
• Early Palliative Care Integration: Proven to improve quality of life, mood, and even survival.

Prognosis

• Stage IV: Median overall survival 6–11 months (with FOLFIRINOX). 5-year OS <5%.
• Locally advanced: Median OS 10–16 months.
• Prognostic factors: Performance status, CA19-9, albumin, metastatic burden, treatment tolerance.
