Ciprofloxacin & Surgery: Timing Risks & Perioperative Management
Ciprofloxacin (a fluoroquinolone) requires careful perioperative timing due to specific risks. Always inform your surgical team of all current medications.
Why Fluoroquinolones Are Concerning Perioperatively
- Tendon Rupture Risk
- FDA Black Box Warning: increased tendinitis/rupture (especially Achilles).
- Risk amplified by: age >60, corticosteroids, renal failure, diabetes, strenuous rehab.
- Surgery itself stresses tendons (positioning, mobilization, bracing).
- Drug Interactions
- CYP1A2 inhibition → elevated levels of theophylline, clozapine, tizanidine, ropivacaine.
- QT prolongation risk with anesthetic agents (propofol, volatile anesthetics), ondansetron, antibiotics.
- Neuromuscular blockade potentiation (rare but reported).
- Impaired Wound Healing
- Animal and in vitro data: fluoroquinolones inhibit collagen synthesis, fibroblast proliferation, TGF-β signaling.
- Clinical reports of delayed closure, dehiscence, anastomotic leak.
Perioperative Recommendations
- Elective surgery: Hold ciprofloxacin ≥48 hours pre-op (5 half-lives); restart post-op when wound stable, oral intake resumed, and no interacting drugs.
- Urgent/emergent surgery: Anesthesiology/surgery/ID consensus on risk-benefit; consider alternative antibiotic if infection prophylaxis needed.
- Document: Indication, last dose, renal function, interacting meds.
Individualized decision-making with your surgeon, anesthesiologist, and infectious disease consultant (if available) optimizes safety.
