Instructions
Register as an organ donor online at the Michigan Department of State's Organ Donor Registry. Enter your Michigan driver's license number or state identification card number, your full name and your date of birth.
Check the box that says "Please send me a replacement sticker," even if this is your first time registering as an organ donor. You will receive a heart sticker indicating your status as a registered organ donor in the mail.
Click "Submit" to add your name to the registry.
Affix the heart sticker to the front of your Michigan driver's license or state identification card.
