Origin:

1) Long head: Infraglenoid tubercle of the scapula

2) Lateral head: Lateral aspect of the humerus (above the radial groove)

3) Medial head: Medial aspect of the humerus (below the radial groove)

Insertion: Olecranon process of the ulna
