North Carolina Birth Certificate: How to Request a Copy

If you are in need of a copy of a North Carolina birth certificate, you may contact the North Carolina Vital Records Office to make your request. The North Carolina Vital Records website has an application that you may print and complete to make a request for a birth certificate copy. You will need to pay a processing fee of $25 to obtain the birth certificate.

      Print the application form.

      Fill the application form with valid information.

      Prepare a check or money order for $24. Make the check or money order payable to the North Carolina Vital Records.
