Medical history:
- Do you have any medical conditions, such as high blood pressure, diabetes, or heart disease?
- Have you had any recent illnesses or injuries?
- Are you currently taking any medications, including over-the-counter medications, herbal supplements, or vitamins?
Social history:
-What is your occupation?
-Do you smoke, drink alcohol, or use recreational drugs?
-Do you have support at home during labor and after the birth of your baby?
Current pregnancy:
- How far along are you in your pregnancy?
- What is your estimated due date?
- Have you been having regular prenatal care?
- Have you noticed any changes in your health or your baby's health?
- Have you had any bleeding, contractions, or leaking of fluid from your vagina?
