What you should do first.
Gather all of your information. To replace your medicare card you will need your full name as it appears on your social security card,your social security number and date of birth.
Visit your local social security office. This is especially important that you do this right away if proof is needed for a prescription or a visit to a doctor. Otherwise it usually takes about four weeks to get a replacement card.
Order a replacement medicare card online. If you have access to the internet this is one of the easiest ways to get your medicare card replaced. To do this you will need to go to socialsecurity.gov or simply visit the link that you will find in the resources section on this page.
If you've moved recently and have not already done so you will need to report this to Social Security. Otherwise your replacement card will be mailed to the address that Social Security has online for you. To do this you can either visit your local Social Security office or you can call the toll free number 1-800-772-1213.
