  Call the treating physician, hospital or supplier. Provide the billing department at the physician̵7;s office with the name of the treating physician, date of service and type of treatment.

Download the Medicare Redetermination Request Form (CMS-20027) if you don't agree with the initial claim decision by Medicare.
