How to Register With Medicare for a Medicare PIN

Medicare issues two different physician identification numbers: the Provider Identifying Number and the Unique Physician Identification Number. Both numbers are issued at the same time and based on the same application, which the physician sends to Medicare. All physicians must obtain a PIN and UPIN, even if they will not bill Medicare directly. To receive a PIN and UPIN, you need to fill out an application form.

      Visit the Centers for Medicare and Medicaid Services website to download the Medicare Enrollment Application -- Physicians and Non-Physician Practitioners form. The form is titled 855I.

      Acquire your billing number, known as an NPI, by visiting the National Plan and Provider Enumeration System website. This is a separate process, but you must have an NPI before applying for your PIN.

      Fill out form 855I, completing the sections that are required depending on your specific occupation. You must use ink and type or print legibly.

      Make a copy of the application packet for your own reference.

      Send in the completed application with original signatures to your designated fee-for-service contractor, also called the Medicare administrative contractor. Each state has a designated Medicare administrative contractor, which can be found on the Medicare and Medicaid Services website.
