Modifier 46 is appended to the primary procedure on a claim to indicate the following, as defined by HCPCS documentation:

-The surgeon performs the surgery with the surgical assistant present at the time.

-The surgical assistant helps the surgeon with the surgery.

-The surgical assistant does not record notes.
