CPT 99215: High-Complexity Established Patient Office Visit

CPT 99215 = Office or other outpatient visit for the evaluation and management of an established patient, which requires a medically appropriate history and/or examination and high level of medical decision making (MDM).

Key Components (2021 E/M Guidelines – MDM Based)

• MDM: High – Must meet 2 of 3:
  • Problems Addressed: ≥1 acute/chronic illness with exacerbation/progression/side effects; OR ≥2 stable chronic illnesses; OR ≥1 acute uncomplicated illness + ≥1 stable chronic; OR ≥1 acute illness requiring hospitalization.
  • Data Reviewed/Analyzed: Extensive (e.g., review prior outside records + order/review multiple tests + discuss with other provider + independent interpretation).
  • Risk: High (e.g., drug toxicity monitoring, decision for major surgery, hospitalization, DNR discussion).
• Time (if used): 40–54 minutes total time on date of encounter.
• History/Exam: "Medically appropriate" – no longer element-counted. Document relevant HPI, ROS, PFSH, focused exam.

Typical Clinical Scenarios

• Uncontrolled diabetes + CKD + new foot ulcer → admit decision.
• New cancer diagnosis discussion + staging workup plan.
• Complex medication management: anticoagulation + multiple interactions + renal dosing.
• CHF exacerbation + renal dysfunction + medication titration.

Billing Nuances

• Established patient only (new patient = 99205).
• Modifier 25 if significant separately identifiable procedure same day.
• Prolonged services (99417) if >54 min.
• Medicare: 2.80 RVU (~$100–$110). Commercial: varies.

Documentation Essentials

• Assessment & Plan: Explicitly state complexity drivers ("high MDM due to…").
• Data: List specific records reviewed, tests ordered/interpreted, providers contacted.
• Risk: Name the high-risk decision (surgery, hospitalization, toxic drug monitoring).
