Go to the Kent County office of the Michigan Department of Human Services, at 121 Franklin, S.E, Grand Rapids; phone, 616-248-1000. Ask the receptionist for DHS-1171, a packet including information and an application for assistance.
Complete the application and include required information, which might include proof of household income, assets and property, personal identification, Social Security number, birth records, marriage certificates or divorce decrees.
Return the application and the additional information to the Department of Human Services in person, by mail or by fax:
