If you cannot readily find a link that will help you find this information, utilize the search function that should be near the top of the website. Enter ̶0;replacement card̶1; into the search box and press "Enter."
Browse through the returns to your search and click on a link that gives you information for requesting a replacement card. You should find a telephone number to call to request this card.
Call the telephone number during regular office hours and provide the identifying information necessary to request a new card.
