Update Your Ontario Health Card Address: A Simple Guide
As a citizen or permanent resident of Canada currently residing in Ontario, you are entitled to free health care services under the Ontario Health Insurance Plan (OHIP). OHIP coverage allows you to have your medical services billed directly to the Ontario Ministry of Health. To receive health care services free of charge, you must present your OHIP card whenever you seek medical care. If you move, you must update your address with the Ministry of Health to keep active medical coverage.Things You'll Need

Change Address Online
Visit Ontario.ca. Select whether you prefer to continue in English or in French.
Select "Services" from the toolbar at the top of the screen.
Scroll down the page until you see "Change Your Home Address." Select this option.
Select "Update a Health Card."
Click the change of address link in the middle of the screen.
Fill out the online change of address form. You will need to provide personal information such as your name, Social Insurance number and date of birth. You must also provide the health number and version code from the front of your OHIP card.
Change Address by Mail
Visit your local Ministry of Health office and request Form 1057-82. This is the official OHIP change of address form.
Fill out the form.
Mail the form to:

Change Address at a Kiosk
Visit a ServiceOntario kiosk near you. ServiceOntario kiosks are commonly located in shopping malls and government buildings.
Select "Change of Address" from the services listed on the ServiceOntario kiosk screen.
Answer each question when prompted. You will need to have your current OHIP card handy to provide the health number and version code.
Submit the electronic change of address form.
