Oregon Health Plan (OHP): Online Enrollment & Eligibility
The Oregon Health Plan (OHP) is a state-sponsored health insurance package currently held by 380,000 Oregon residents. Members of the OHP must be low-income, non-pregnant Oregon residents aged 19 years and older. OHP premiums vary based on the needs of each individual, with some members receiving full health benefits at no cost. To enroll in the OHP, residents must sign up for the Oregon Department of Human Services' reservation list.
Instructions
Fill out the Oregon Health Plan Standard Reservation List form online, which is available on the State of Oregon's website. You may fill out an Oregon Health Plan reservation for yourself and up to two non-pregnant family members who are 19 or older. Click the "Save" button to submit your name to the Oregon Health Plan reservation list.
Write down your Oregon Health Plan reservation number, which will be mailed to you in a letter from the Oregon Department of Human Services once your reservation form is submitted.
If you are selected from the reservation list, the Oregon Department of Human Services will mail you an application to the Oregon Health Plan. Fill out the application in print or online to determine your eligibility for the program.
