PeachCare for Kids: Enrollment, Eligibility & Benefits in Georgia
PeachCare for Kids provides free or low-cost healthcare insurance for children under the age of 19 in the state of Georgia. Only uninsured children can receive PeachCare, and their families must meet specific income guidelines. PeachCare for Kids covers services such as out-patient and in-patient medical care, preventative healthcare, dental care, vision care, prescription drugs and mental health care.Things You'll Need

Instructions
Apply online by visiting the PeachCare for Kids website or by downloading an application from the site.
Complete the application. You must provide information about your child; all sources of income; your employer, if you currently have a job; any prior health insurance coverage information; and any daycare expenses you pay.
Provide copies of documents verifying your household income, including your last four paycheck stubs, if you work; an award letter from Social Security, if you receive disability; unemployment-benefit payment stubs, if you receive unemployment; and court documents showing how much child support you receive, if any.
Mail the completed application and the necessary documentation to PeachCare for Kids, P.O. Box 2583, Atlanta, GA 30301-2583. PeachCare will not accept faxed applications.
