Remove Spouse from Health Insurance: A Step-by-Step Guide
The more beneficiaries on your health insurance policy, the higher the cost. To reduce the cost, you may remove beneficiaries, including your husband, from your plan. Before removing your husband from your health insurance policy, allow him ample time, perhaps a couple of weeks, to find a new insurance plan.
Instructions
Call your health insurance's customer service department or agent.
Request to remove your husband from your health insurance policy. Submit your full name, policy number or Social Security number, date of birth and mailing address. If necessary, the representative will send a form in the mail that you must complete to amend the policy. You may also need to select a new plan, if the current policy is for families and removing your husband will leave only you on the policy. The representative will go over your plan options and charges.
Complete the form and return it by mail or fax. Once the form is processed, your husband is removed from the policy.
