How to Replace an Ontario Health Card
The national health care system is one of the prized features of Canadian society. Every resident of Canada receives a health card that he must present to get medical services. There are some situations in which you may need a replacement card -- for example, if you lose your card, if someone steals it or if it gets damaged. Gather your documents. You need three documents to prove Ontario residency: one that shows you are a Canadian citizen; one that shows you are an Ontario resident and one that proves your identity. Gather any additional documents, such as a marriage certificate or a change of name certificate. Look at the official form in Resource 1 for a complete list of acceptable documents.
Go to an Ontario Health Insurance Provider office to apply for a replacement card.
Explain that you need a replacement health card. Provide all your documents to the staff person at the desk. Answer any questions. The staff person will fill out the necessary forms for you.
Accept the new card if the office is able to print one out for your. Otherwise, wait for the new card. It should arrive in the mail within a few days to a week.
