Tennessee Nursing License Address Change: A Step-by-Step Guide
In the state of Tennessee, nurses must notify the Board of Health of any address changes within 30 days. Once the Board of Health is notified, the change will be updated in the system with the practice and mailing address made available to the general public. There are several options available from the Tennessee Board of Health to change the address on a nursing license.

Complete the name/address change form available on the Tennessee Health Board's website. Once you have filled out the form, print it out, sign it and either fax or mail the form.

Change your address online using the Tennessee Anytime website. Note that while you can change your address online, your name cannot be changed with this option.
