How to Locate NPI
The National Provider Identifier is an ID health care providers use that will be used by all health plans, making it a standard number nationwide. NPIs are 10 digit numbers given to health care providers who submit claims or perform other services as specified by HIPAA. If you need to find a provider's NPI number, a quick search should provide you with the information.

NPI Database
Visit npidb.org/npi. This site allows you to search by doctor or organization.
Enter the first and last name of the doctor you're looking for. Also enter the state the doctor practices in.
Choose your sort: "Last, First Name," "First, Last Name" or "State, City." Click "Search."
Search by organization. If you don't know the doctor's name, click on "NPI Lookup for Organizations" and enter the organization name and state, and click "Search."
NPI Registry Search
Visit nppes.cms.hhs.gov/NPPES/NPIRegistrySearch.do?subAction=reset&searchType=ind.
Enter the provider's first or last name, and the city or state in which they practice. Click "Search."
Click "Back" to search by organization. Click "Organizational Provider."
Enter the organization's name or the "Doing Business As" name. Click "Search."
