Locate the medical facility your spouse has visited. Obtain their phone number, mailing address or fax number.
Call the facility, reach the medical records department, and request that they e-mail, fax, or mail you a HIPAA-compliant release form.
Ask your spouse to fill out and sign the release form. Fax or mail the completed form back to the health facility, including where the records should be sent.
Wait one to four weeks to receive all records. Some facilities may fax records on the same day; others may use postal mail.
