How to Apply for Healthy Families
Healthy Families is a program available in the state of California to provide children with a low cost form of health care coverage. If your child does not qualify for free children's health insurance from California Medi-Cal, and she is not currently covered, you might be able to sign her up for Healthy Families. This program will cover medical, dental and vision for qualifying children.Things You'll Need

- Computer with Internet access
- Computer printer
- 1040 Income Tax Return
- Proof of citizenship and proof of residency in California
- Envelope included with application
Instructions
Download an application from the Healthy Families website or request an application by calling (800) 880-5305 Monday through Friday between 8am and 8pm. Fill out the application completely.
Obtain copies of papers showing proof of income including a copy of your 1040 from the previous year. Send copies of proof of citizenship and proof of residency in California. Acceptable forms of proof include a birth certificate or a utility bill.
Seal the application and copies of the necessary paperwork in the envelope included with the application.
Address the envelope to: Healthy Families/Medi-Cal, P.O. Box 138005, Sacramento, CA 95813-9984.
