Smooth surface caries does not begin on the outer of enamel and dentin. It typically starts in areas where plaque can easily accumulate and is less accessible to regular brushing and flossing, such as the pits and fissures on the chewing surfaces of the back teeth (molars and premolars) and in between teeth.
