Blood type is determined by the presence or absence of certain antigens on the surface of red blood cells. People with O negative blood do not have any of these antigens, while people with O positive blood have the A antigen. If an O positive person donates a kidney to an O negative recipient, the recipient's immune system will recognize the A antigen on the donor's kidney cells as foreign and attack them, leading to rejection of the kidney.
