How to Create a Personal Medical Profile
Personal medical profiles can tell other people a lot about your medical history and conditions. They can save your life in an accident or be a helpful tool when visiting a new doctor. Many people do not wear a medical ID alert bracelet because it advertises that they have a medical condition. A USB drive containing all of your important medical information can be put on a lanyard and worn around your wrist or neck or kept close by.Things You'll Need

- Lanyard or chain
Instructions
Create word processing document
Open a word processing program on your computer. Open a new document.
Type in your name, address and contact information.
List any medical conditions in bold capitals or a contrasting ink color.
List all of your medical prescriptions that you take regularly, their dosages and when you take them.
Put in your doctor's name and telephone number.
Type in the name and telephone number of a person you can use as an emergency contact.
List any health insurance information you would like to have in your personal medical profile. Save your document to the computer desktop.
Upload to USB drive
Plug your USB drive into your computer. Wait for your computer to recognize the USB drive.
Click on the USB drive icon to open it.
Drag and drop your personal medical profile document into the USB drive window.
Eject the USB drive by dragging it and dropping it into the trash.
Place an outside label on the USB drive indicating that it holds medical information. Attach a chain or lanyard to keep it on your person.
