Visit www.socialsecurity.gov, Apply for Disability Benefits for Child (see resources).
Review the Child Disability Starter Kit. This includes a checklist and worksheet that will help you locate the child's medical and personal information. Your child's medical history, medications, school records and your current income, among other things, will be required during an interview.
Contact the SSI office by phone (800-772-1213) to schedule an interview in person or over the phone.
Fill out the Child Disability Report online. Sign the form to authorize your child's doctor to present medical information about the child.
Wait three to five months for a decision. You will receive the decision by mail.
