  Launch your Internet browser. Navigate to the Abbott Diabetes Care registration page.

  Input the date you purchased your Freestyle meter by selecting the proper date in the drop-down menus.

  Enter your name, the serial number of your meter and your street address in the fields provided. Select the type of Freestyle meter you have in the drop-down menu. Provide your phone number if you want to.

  Add your e-mail address where prompted. Populate the drop-down fields with the method you use to control your diabetes, the testing frequency, your birth date, diabetes diagnosis information and your gender.

  Click "Submit" to process the device registration.
