Dexmedetomidine (Precedex) Protocol: Intensive Care Sedation

Dexmedetomidine or Precedex is used to sedate patients in intensive care. The drug leaves them conscious and able to communicate. The medication is given as a continuous infusion and also helps control anxiety and pain.

Loading Infusion

• Precedex therapy typically begins with a loading infusion delivered at a rate of 1 microgram/2.2 lbs/10 minutes. A loading dose may not be required in patients who are already sedated. If Precedex is to be given with other medication which have a sedative effect, dosages of each may need to be lowered.

Maintenamce Dose

• The infusion rate of Precedex should be adjusted individually to produce the desired effect. The recommended maintenance dose in ICU is 0.2 to 0.7 micrograms/2.2 lbs/hr and 0.2 to 1.0 mcg/2.2 lbs/hr for sedation prior to surgery or other procedures.

Other Recommendations

• Doses may need to be lowered for patients with liver or kidney problems, as well as patients older than 65. Precedex is not associated with depression of respiration, unlike other strong sedatives.
