Gender-Affirming Surgery: Process, Procedures & What to Expect
Terminology Note
“Transsexual” is outdated. Current terms: transgender, gender-diverse, non-binary. Surgery = gender-affirming surgery (GAS) or gender-confirming surgery.
Step 1: Pre-Operative Evaluation (WPATH SOC-8 Standards)
- Mental Health Assessment: Persistent, well-documented gender incongruence; capacity to consent; mental health stability (not “clearance” but collaborative).
- Hormone Therapy: Typically ≥12 months (unless medically contraindicated) for genital surgery; not required for all procedures.
- Medical Optimization: Primary care evaluation, labs, smoking cessation (critical for healing), BMI optimization.
- Surgical Consultation: Anatomy review, goals, procedure options, risks, recovery, expected outcomes, photos.
- Insurance/Financial Clearance: Prior auth, letters of support, cost estimates.
Step 2: Common Surgical Procedures
| Goal | Procedures |
|---|---|
| Transfeminine (AMAB → Female) | |
| Genital | Vaginoplasty (penile inversion, peritoneal pull-through, sigmoid colon), vulvoplasty (no vaginal canal), orchiectomy (standalone) |
| Breast | Augmentation (implants/fat grafting) if HRT insufficient |
| Facial (FFS) | Forehead contouring, brow lift, rhinoplasty, jaw/chin contouring, tracheal shave (Adam’s apple reduction), lip lift, hairline advancement |
| Voice | Glottoplasty, feminization laryngoplasty |
| Body | Liposuction, fat grafting (hip/buttock), breast augmentation |
| Transmasculine (AFAB → Male) | |
| Chest | Mastectomy (top surgery) – double incision, periareolar, keyhole |
| Genital | Metoidioplasty (clitoral release), phalloplasty (radial forearm, ALT, musculocutaneous flap), scrotoplasty, testicular implants, urethral lengthening |
| Facial | Less common; jaw augmentation, rhinoplasty |
| Voice | Testosterone usually sufficient; rarely thyroplasty |
Step 3: Post-Operative Care
- Hospital stay: 1–5 days (vaginoplasty/phalloplasty longer).
- Pain management, drains, catheters, wound care, dilation (vaginoplasty – lifelong).
- Activity restrictions: 4–6 weeks no heavy lifting; 6–12 weeks no strenuous exercise.
- Hormone continuation (adjusted per surgery).
- Mental health support (transition adjustment).
Step 4: Long-Term Follow-Up
- Surgeon: 1 wk, 6 wk, 3 mo, 6 mo, 1 yr, then annually.
- Primary care: Hormone monitoring, preventive screening (based on anatomy present).
- Pelvic floor PT (vaginoplasty).
- Complication surveillance: stenosis, fistula, urethral stricture, implant issues.
Key Considerations
- Not all transgender people want/need surgery.
- Cost: $20k–$150k+; insurance coverage varies.
- Surgeon selection: Fellowship-trained, high volume, portfolio, patient reviews.
- Legal name/gender marker change – separate process.
Bottom Line
Gender-affirming surgery is a multi-step, multidisciplinary process centered on informed consent, medical optimization, and individualized surgical planning. Outcomes are highly satisfying when criteria are met and expectations are realistic.
