Does the stylus mark paper by impulse on ekg?

The stylus of an electrocardiogram (ECG) machine does not mark paper by impulse. Instead, it uses a heated element to burn a trace onto special ECG paper. The ECG machine records the electrical activity of the heart and amplifies it so that the trace can be seen on the paper.
